Subclinical hypothyroidism: a 'laboratory-induced' condition?

Een te hoge TSH met een nog ‘normale’ FT4 wordt een subklinische hypothyreoïdie genoemd.
Niet iedereen heeft dan klachten, terwijl er in het laboratorium toch een subklinische hypothyreoïdie te zien is.
Sommige te hoge TSH’s worden niet aangepakt omdat er niet altijd sprake is van een schildklierziekte.
Zo kan iemand die ernstig ziek is, tijdelijk te hoge TSH’s hebben, terwijl de FT4 normaal is.
De Nederlandse richtlijnen zetten de grens voor behandelen, afhankelijk van de situatie, bij ongeveer een TSH van > 10 mU/l.

Er zijn grote verschillen van normaalwaarden per laboratorium.
In het ene lab is soms een TSH van 4 mU/l verhoogd, als de normaalwaarden bijvoorbeeld tussen de 0.3-3 mU/l liggen.
In een ander lab kan een TSH van 4 mU/l ‘gezond’ zijn, als de normaalwaarden tussen de 0.4 - 4.5 mU/l blijken te zijn.
Zo geldt dat ook voor subklinische hyPERwaarden.

Hoe is de verdeling van (sub)klinische schildklierziekten in de Nederlandse laboratoria?
Je zou denken dat het een evenredige verdeling zou geven per lab.
Maar dat blijkt niet zo te zijn.
In het ene lab komen veel meer schildklierziekten voor dan in het andere lab.
Het ene meetproduct geeft lagere of hogere cijfers dan het andere meetproduct.

Dit onderzoek laat zien dat er grote verschillen bestaan tussen het ene lab en het andere.
Met name in het gebied van de subklinische hypothyreoïdie pieken de verschillen.

Van lab switchen is dus ongewenst, als je niet beseft wat voor gevolgen dit kan hebben.
Het zou goed zijn als men de meetapparatuur laat ‘ijken’ door het al onderzochte bloed ook door een collega-lab te laten meten.

Mevrouw Coene pleit in dit onderzoek voor de noodzaak van harmonisatie.
Ieder lab dezelfde meetspullen.
Ook internationale organisaties op dit gebied erkennen deze noodzaak.

Abstract
OBJECTIVE:

In current literature and guidelines, there is a tendency to define absolute TSH concentrations at which patient follow-up or even pharmaceutical intervention should be initiated. As TSH concentrations depend on the analytical method/platform used for TSH quantification, absolute cut-off values may pose threats for uniform clinical decision-making. In this study we therefore set out to clarify to what extent the method/platform and the reference values applied for TSH influence the clinical interpretation of thyroid parameters.
DESIGN AND METHODS:

We retrospectively analyzed anonymous TSH results from the Dutch external quality assessment program (EQAS) in relation to reference values advised by different manufacturers. We also examined TSH/free thyroxin (fT4) reference ranges and prevalence of thyroid pathology among different Dutch laboratories, including four cases in which a switch in the measuring platform was made.
RESULTS:

Our data show that interpretation of thyroid parameters is not only influenced by between-method/platform variation, but is also substantially affected by the variation in TSH/fT4 reference intervals applied in individual laboratories. Additionally, we show that the transition to a novel analytical method/platform can result in a shift in the prevalence of thyroid pathology, especially for subclinical hypothyroidism.
CONCLUSIONS:

Subclinical hypothyroidism can be a 'laboratory-induced' condition. This is an undesirable situation in regard to the clinical implications such a diagnosis can have for patients.

Standaardisatie van schildklierfunctie tests gewenst

Betrouwbare laboratoriumtests voor serum TSH en vrije schildklierhormonen zijn onmisbaar voor huisartsen en endocrinologen. Bloedonderzoek speelt namelijk een grote rol bij de diagnose en behandeling van schildklieraandoeningen.

Het belang van standaardisatie van schildklierfunctie tests is erkend door laboratoria wereldwijd. Alleen door standaardisatie kan men de meetresultaten tussen de methoden onderling vergelijken. Dit geldt in het bijzonder voor tests voor vrij T4 (FT4), die nu controversieel zijn in termen van klinische en analytische validiteit. Maar ook bij het testen van de TSH is sprake van variatie. Iets wat veel meer aandacht verdient in de lopende discussies om de bovengrens van TSH-referentiewaarde te verlagen, waardoor de behandeling van hypothyreoïdie eerder zou kunnen beginnen.

Het Comité voor normalisatie van de schildklierfunctie tests van Internationale Federatie van Klinische Chemie en Laboratoriumgeneeskunde (IFCC) werkt inmiddels aan de standaardisatie van FT4 en TSH testen.

Oproep
Met de oproep voor endocrinologen en andere artsen wil het comité de opvattingen peilen over de voordelen, risico's en implementatie van standaardisatie van schildklierfunctie tests. Deze oproep is gestuurd naar verschillende klinische tijdschriften en professionele organisaties voor artsen en laboratoria specialisten.

Tot slot
Op schildklierfora kun je lezen hoe al deze verschillende meetmethoden voor verwarring zorgen bij behandelaars en patiënten, zodra het lab weer eens een ander apparaat aangeschaft heeft met andere referentiewaarden.
