Abstract

Background
Subclinical hypothyroidism (SCH) is a common condition in elderly people, defined as elevated serum thyroid-stimulating hormone (TSH) with normal circulating free thyroxine (fT4). Evidence is lacking about the effect of thyroid hormone treatment. We describe the protocol of a large randomised controlled trial (RCT) of Levothyroxine treatment for SCH.

Methods
Participants are community-dwelling subjects aged ≥65 years with SCH, diagnosed by elevated TSH levels (≥4.6 and ≤19.9 mU/L) on a minimum of two measures ≥ three months apart, with fT4 levels within laboratory reference range. The study is a randomised double-blind placebo-controlled parallel group trial, starting with levothyroxine 50 micrograms daily (25 micrograms in subjects <50Kg body weight or known coronary heart disease) with titration of dose in the active treatment group according to TSH level, and a mock titration in the placebo group. The primary outcomes are changes in two domains (hypothyroid symptoms and fatigue / vitality) on the thyroid-related quality of life questionnaire (ThyPRO) at one year. The study has 80% power (at p = 0.025, 2-tailed) to detect a change with levothyroxine treatment of 3.0% on the hypothyroid scale and 4.1% on the fatigue / vitality scale with a total target sample size of 750 patients.

Secondary outcomes include general health-related quality of life (EuroQol), fatal and non-fatal cardiovascular events, handgrip strength, executive cognitive function (Letter Digit Coding Test), basic and instrumental activities of daily living, haemoglobin, blood pressure, weight, body mass index and waist circumference. Patients are monitored for specific adverse events of interest including incident atrial fibrillation, heart failure and bone fracture.

Discussion
This large multicentre RCT of levothyroxine treatment of subclinical hypothyroidism is powered to detect clinically relevant change in symptoms / quality of life and is likely to be highly influential in guiding treatment of this common condition.

Study population
The trial is running in four countries (UK, Ireland, the Netherlands and Switzerland). Potential subjects are identified from clinical laboratory databases and recruited from the community, aged ≥65 years with SCH, diagnosed on the basis of persistently elevated TSH levels (4.6–19.9 mU/L) with fT4 within the reference range. Baseline and follow-up TSH was measured by clinical laboratories affiliated with each clinical centre. TSH is measured on a minimum of two occasions at least three months apart, over a maximum period of 3 years. All subjects gave individual informed consent to participate (See Appendix 1 and 2; Additional files 1 and 2 for model patient information sheet and consent forms).

Exclusion criteria for the study:
i) Subjects currently prescribed Levothyroxine, anti-thyroid drugs, amiodarone or lithium.
ii) Recent thyroid surgery or radio-iodine (within 12 months).
iii) Grade IV NYHA heart failure.
iv) Clinical diagnosis of dementia.
v) Recent hospitalisation for major illness or elective surgery (within 4 weeks).
vi) Recent acute coronary syndrome, including myocardial infarction or unstable angina (within 4 weeks).
vii) Acute myocarditis or pancarditis.
viii) Untreated adrenal insufficiency or adrenal disorder.
ix) Terminal illness.
x) Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.
xi) Subjects participating in ongoing RCTs of therapeutic interventions (including Clinical Trials of Investigational Medicinal Products; CTIMPs).
xii) Plan to move out of the region in which the trial is being conducted within the next 2 years.

The dose is changed according to the serum TSH level as follows; 6–8 weeks after randomisation a blood sample is taken for serum TSH, with three possible actions:

1.TSH <0.4 mU/L: treatment dose reduced to 25 micrograms levothyroxine in those starting on 50 micrograms; reduced to 0 in those starting on 25 micrograms – blinding maintained by giving placebo matching the 25 micrograms dose; these patients will have a further check TSH after 6–8 weeks; if TSH remains <0.4 mU/L patient will be withdrawn from randomised treatment.

2.TSH ≥0.4 and <4.6 mU/L: no change to the treatment dose; patient to be reviewed at 12 months.

3.TSH remains elevated (≥4.6 mU/L): additional 25 micrograms levothyroxine, giving a total daily dose of 75 micrograms levothyroxine for those starting on 50 micrograms, or a total daily dose of 50 micrograms levothyroxine for those starting on 25 micrograms

Ouderen hoeven blijkbaar niet behandeld te worden.
Ze zijn toch al moe. Laat ze maar.
Hoe ouder hoe minder weerwoord, hoe makkelijker en goedkoper.

The recently published TRUST trial (NEJM, April 3, 2017) was a placebo-controlled randomized trial of thyroid hormone replacement among 737 older adults with subclinical hypothyroidism. After 1–3 yr of follow-up, the overall results found no beneficial effects on self-reported hypothyroid symptoms or tiredness.

The effects of thyroid replacement among TRUST participants with more baseline hypothyroid symptoms and tiredness, who might benefit to a greater extent than those with fewer symptoms, have not been reported. TRUST randomized adults ‡ 65 years old with untreated persistent subclinical hypothyroidism (TSH 4.6 to 19.0 mIU/L) to either levothyroxine or matching placebo.

368 were assigned to a levothyroxine starting dose of 50 lg/d (or 25l gifweight < 50Kg or coronary heart disease) with blinded dose titration to reach a TSH < 4.6 mU/L, and 369 received matching placebo with mock dose titration.

The 2 primary outcomes were 1 yr. change in Hypothyroid Symptoms and Tiredness scores (range for each scale 0-100 with 9 point difference considered clinically significant). In this analysis we examine the effects of treatment vs. placebo on the primary outcomes among participants with higher (i.e. worse) baseline symptom scores.

Mean (-SD) Hypothyroid Symptom scores at baseline were 18–19 and 17–18 in the
thyroxine and placebo groups, respectively, and over 1 yr. mean symptom scores fell by 0.2 in both groups (between group difference 0.0, 95% confidence interval: - 2.0 to 2.1). Similar to the overall results, we found no evidence that baseline Hypothyroid Symptom scores were associated with the effects of thyroxine vs. placebo on symptom scores (interaction p-value = 0.85). For example, among those with baseline Hypothyroid Symptom scores >30 (approximately the upper quintile, N= 132), mean Hypothyroid Symptom scores fell from 45 – 14 to 33 -21 after 1 yr. in the thyroxine group, and fell from 45 – 15 to 35 – 21 in the placebo group.

Results were similar among those with greater baseline Tiredness scores (interaction p-value = 0.65) Levothyroxine replacement did not improve Hypothyroid Symptoms or Tiredness in older persons with subclinical hypothyroidism, including those with higher symptom scores at baseline.

VIP sinds mijn geboorte, diagnose Hashimoto maart 2008
Ex-Thyrax, geswitcht naar Eutyrox, zeer tevreden.
In stapjes van 12,5 van 175 naar 125 mcg (licht hyperen/staken pil).
Ook vit B12- en D-tekort, 2x/week een B12-injectie sinds 15-03-2017.

Het zijn dezelfde onderzoekers, Suffy.
Ik vind het een schandalige ontwikkeling.

Analyse van het menselijk brein met een vragenlijst, wat een verfoeilijke toepassing van statistiek.
En dan beweren de medisch onderzoekers ook nog dat zo'n 'statistisch wiskundige analyse' de meest betrouwbare en objectieve antwoorden geeft.
Schoenmaker, blijf bij je leest!, zou je ze willen toeroepen.

Het lijkt dat de onderzoekers de gewenste politieke richting volgen: goedkopere zorg voor ouderen.
De aanzwellende 'grijze golf' is immers al jaren een grote zorg.
Er lijkt veel geld beschikbaar te zijn voor dit soort modieus onderzoek.

Nu dreigt het gevaar, of het is al besloten, dat de medische richtlijnen worden aangepast: alle ouderen boven de 70, met een nieuwe hypothyreoïdie, gezond of niet gezond, niet meer substitueren.
En de arts verschuilt zich achter de richtlijn: Ik kan er niets aan doen, mevrouw/meneer, het staat in de richtlijn! En u heeft er toch geen last van!

hallo Kiek en Suffy, op Schildkliertje heb ik dit begin april geplaatst: TRUST: vervolg onderzoek behandeling met levothyroxine bij ouderen zorgt voor misverstanden.

'Het doel van het TRUST-onderzoek was om te onderzoeken wat de voor- en nadelen zijn van behandeling van schildklierhormoon bij ouderen met licht verminderde schildklierwerking. Het onderzoek was specifiek gericht op het voorkómen van hart- en vaatziekten, én het verbeteren van kwaliteit van leven (bijvoorbeeld door verminderen van vermoeidheidsklachten), spierkracht en denkvermogen. De conclusie dat behandeling niet zinvol zou zijn is wel heel kort door de bocht.

In dit artikel van de NEJM wordt het TRUST-onderzoek uitgebreid besproken. De conclusie van het onderzoek is dat de symptomen niet verbeteren bij het gebruik van levothyroxine door ouderen met subklinische hypothyreoïdie. De onderzoekers houden enkele slagen om de arm (lees afbeelding). Inmiddels leidt de conclusie al het eigen leven dat ouderen met een subklinische hypothyreoïdie geen levothyroxie nodig zouden hebben.'

Geen suppletie voor ouderen met subklinische hypothyreoidie

Acht tot 16% van de mensen boven de 65 jaar heeft tekenen van subklinische hypothyreoïdie. Dit gegeven is eerder in verband gebracht met verschillende symptomen en chronische ziekten, maar er is geen bewijs dat suppletie met levothyroxine zin heeft. Recent onderzoek laat zien dat suppletie geen effect heeft op symptomen en andere klinisch relevante uitkomsten.

Een internationale groep onderzoekers, waaronder Nederlandse, onderzocht bij 737 patiënten met subklinische hypothyreoïdie of suppletie effect had op symptomen van hypothyreoïdie. De gemiddelde leeftijd van de patiënten was 74 jaar. De onderzoekers definieerden hypothyreoïdie als een verhoogde TSH-waarde (4,60 tot 19,99 mU/l) en een normaal vrij T4. Patiënten die reeds suppletie kregen, werden uitgesloten van het onderzoek.

De deelnemers kregen ofwel suppletie met een startdosering van 50 microg levothyroxine per dag, dat werd opgehoogd tot een TSH van 0,40 tot 4,59 mU/l bereikt was, ofwel placebo. Na 12 maanden was de TSH-waarde in de suppletiegroep gemiddeld 1,92 mU/l lager dan in de placebogroep. Er was echter geen verschil in de hypothyreoïdiesymptoomscore (verschil 0,0; 95%-BI –2,0 tot 2,1) of moeheidsscore (verschil 0,4; 95%-BI –2,1 tot 2,9). Ook bij patiënten met een hoog TSH (> 7 mU/l) voor de start van de suppletie was er geen effect op de symptomen. Er was evenmin verschil in bijwerkingen of in het optreden van hart- en vaatziekten tussen de groepen.

Dit onderzoek maakt duidelijk dat suppletie met levothyroxine bij ouderen met subklinische hypothyreoïdie niet beter werkt dan placebo om symptomen te verlichten. De huidige NHG-Standaard Schildklieraandoeningen beveelt aan om subklinische hypothyreoïdie in principe niet te behandelen. Alleen bij persisterende klachten en een TSH hoger dan 6 mU/l kan de huisarts een proefbehandeling overwegen. Dit onderzoek laat zien dat deze aanbeveling terecht zeer terughoudend is en misschien nog terughoudender kan.

Over TRUST trof ik zojuist bij toeval de reacties van een vijftal onderzoeksgoepen.
De reactie van Hannah-Shmouni en Soldin vind ik de belangrijkste:

Waarom gebruik je achterhaalde meetapparaten die buiten de referentiewaarden zo slordig meten?
Soldin pleit terecht al jaren voor een betere wijze van meten.

En dan is het commentaar van de auteurs van het TRUST-artikel:
....is interesting but remains to be proven.
....moet je eerst maar eens bewijzen.
