Hoezo dus? Waar kan ik die bevestiging lezen?

Ik lees in de laatste alinea: Currently, TSH is generally used to adjust thyroid hormone treatment, but if these data are confirmed, it might mean that T4 levels should be routinely measured, too. For now, it should make us cautious about treating mild subclinical hypothyroidism, particularly among older adults at higher risk for AF.

(Momenteel wordt TSH over het algemeen gebruikt om de behandeling van schildklierhormonen aan te passen, maar als deze gegevens worden bevestigd, kan dit betekenen dat T4-niveaus ook routinematig moeten worden gemeten. Omdat dat er gewoon staat - dus niet per se mijn woorden - en niet alleen in dit "verhaaltje".

En dan benadrukken dat vervolgonderzoek nodig is, als je telkenmale vergelijkbare onderzoeken zonder de nodige inhoudelijke reactie, maar de prullenbak verwijst, vind ik een beetje lachwekkend.

Ik hoef helemaal geen extra onderzoek.
Dat kon je ook lezen in mijn vorige bericht

Dat extra onderzoek willen de onderzoekers.
Dat kun je lezen in elk artikel, elk onderzoek en elke conclusie.

Als je op internet kijkt is men al decennialang bezig met onderzoek naar schildklier en AF.
En de conclusie luidt telkens dat meer onderzoek nodig is.

Reageer gewoon inhoudelijk op de verschillende onderzoeken, ipv voor de zoveelste keer op de man,om mij maar in kwaad daglicht te proberen te stellen.

Wat is er goed, wat kan beter?

Zonder dit soort onderzoeken slikte je nu geen levothyroxine, besef je dat?

Looking back at the 2015 Rotterdam study (correctly interpreted in the referenced Medscape article) that study was ALSO looking at the effects of high FT4 in the subset of people with atrial fibrillation in a large population. They specifically EXCLUDED those on thyroid replacement therapy for the particular analysis of all cause mortality. So there, again, what was being studied was the association of high FT4 with development of AF and all cause mortality risk...in presumed hypERthyroidism, and NOT in overtreated hypOthyroidism

There has been some confusion regarding this article and I wanted to provide some clarification. The actual study was performed in individuals in a healthcare electronics database system who were not receiving any thyroid hormone replacement therapy at all. The researchers found that the 'high' end of normal thyroxine levels in these people was associated with a 40% higher risk of atrial fibrillation compared with the 'lowest' end of normal (highest quartile compared with lowest quartile). They thus extrapolated this to indicate that if people being treated for hypothyroidism with levothyroxine had their dose pushed a little too high, so that their thyroxine was in the 'high' normal range, they could be at risk for AF.
However, importantly, they themselves, and the outside commentator, stress that this still remains to be demonstrated in a study in people actually taking levothyroxine for hypothyroidism.

This story is NOT about hyperthyroidism.

It's well known that T4 is the inactive form of thyroid hormone. It needs some enzyme action to modify its iodine to become T3, the active form. So it's very interesting, if it's true, that the "inactive" form can effect the heart so drastically when the active form does not.

@Jeffrey Patten They should have correlated the FT3 to the high/low T4 If T3 is Low and T4 at high end of normal the patient may be missing necessary ingredient to make that conversion. Since these pts were NOT on thyroid medication I assume their TSH was normal. If weight gain and fatigue were the reason for being labeled Sub clinical hypothyroid , then Magnesium deficiency could be a problem. Magnesium also drives metabolism.

T4 to T3 conversion needs B1 B2 B6 zinc and selenium. Plus Tyrosine and maybe L glutathione/aspartic to be normal

I always want to know how the people were paid that do these studies. I for one know that treating people with hypothyroidism, sub clinical, has made a world of difference in their lives. I have used T3 and T4. Not once have any of my patients ever developed atrial fibrillation

Free T4 is not a very good test. It should only be used as a followup to abnormal TSH. If it is high or low under these conditions (after TSH). the likelihood it is correct is high. Each Manufacturer has their own method and generally they have not been clinically validated but simply correlated with another Manufacturer's method. Only a few very complex methods such as equilibrium dialysis are valid methods. The first rapid Manufacture's methods may have been correlated against equilibrium dialysis, I can't remember. In any case the valid free T4s are hardly run any more and definitely not in routine Labs. No information about the methods was given. The Author of the review is a clinician that most likely knows nothing about methods. In my opinion, the study is most likely hogwash.

The combination of all available large prospective cohorts with 25 378 participants suggests that the risk of HF increased both with lower and higher TSH levels, particularly in those with TSH levels 10.0mIU/L and in those with TSH 0.10 mIU/L.

Our findings contribute to a better interpretation of TSH levels in the prevention and investigation of HF.
Pending results from randomized controlled trials, the findings of our study might be useful to define the TSH threshold for thyroid medication among participants with subclinical thyroid dysfunction, although clinical decisions based only on observational studies should be made with great caution because they are subject to limitations.
No clinical trial has assessed yet whether treating subclinical hypothyroidism improved HF outcome. Clinical Significance
• Subclinical thyroid disease is common, and its prevalence increases with age. As the population ages, the prevalence of subclinical thyroid disease is expected to increase.
• The management of subclinical thyroid disease is questionable, particularly in the elderly. Several studies have reported decreased mortality associated with this condition in the elderly, and others have reported some adverse outcomes associated with subclinical thyroid disease.
• This is a large-scale study performed in a cohort of elderly individuals in whom the association between mortality and subclinical thyroid disease was evaluated.
• The increased mortality associated with both subclinical hypothyroidism and hyperthyroidism and the cutoff of thyroid-stimulating hormone values in which mortality was found to be increased are 2 new findings that we believe supply important information for clinicians and may influence therapeutic decisions in patients in whom the decision to treat is not obvious.

The association between subclinical thyroid disease and mortality in the elderly may be even more difficult to define because of the presence of multiple comorbidities and because the normal thyroid-stimulating hormone and free T4 values may be higher in the elderly and are not clearly defined

This study is unique because individuals treated with all forms of treatment modalities used for the treatment of subclinical thyroid disease were excluded from the analysis. Thus, the association between subclinical thyroid disease and mortality was evaluated regardless of the influence of treatment on mortality.
Because thionamides have been reported to be associated with significant side effects, including fulminant hepatic failure,26 and radioiodine therapy has been reported to be associated with mortality in those not rendered hypothyroid by treatment,27 we believe that elimination of these treatment modalities from the analysis was the most accurate way to evaluate the association between subclinical thyroid disease and mortality.
To minimize violation of conditioning on the future, we performed an analysis on the entire cohort that revealed an association of subclinical hypothyroidism and hyperthyroidism with mortality, although this association was more significantly influenced by this analysis for subclinical hyperthyroidism compared with subclinical hypothyroidism. It is possible that physicians tend to prescribe medications earlier to those with subclinical hyperthyroidism than to those with subclinical hypothyroidism, particularly in the older age group. In addition, a significant proportion of individuals had a repeat thyroid-stimulating hormone evaluation performed.
This may have been due to the fact that treatment was not given to these individuals, and this may have caused their primary care physician to follow them more closely. Because thyroid-stimulating hormone values were reproducible in most individuals with subclinical thyroid disease, we believe that this cohort represents individuals with true subclinical thyroid disease and that these results accurately represent the outcome of these conditions.

Conclusions
Subclinical thyroid disease, both subclinical hypothyroidism and subclinical hyperthyroidism, is associated with increased mortality in the elderly. Subclinical hypothyroidism has a thyroid-stimulating hormone threshold value beyond which an increased mortality is noted and mortality is highest at thyroid-stimulating hormone values greater than 6.38 mIU/L, whereas no such threshold value is present in subclinical hyperthyroidism, in which increased mortality is noted across all thyroid-stimulating hormone values.

Beste Ineke,

Het gaat me in zekere zin vooral ook om de manier waarop een onderzoek of onderzoeken als zinloos en niet nuttig worden weggezet. Vooral als de uitkomst niet gewenst is of lijkt te zijn.
Daarom was ik verbaasd dat Laura dat commentaar zelf op haar eigen FB-pagina plaatste. Omdat het op een bepaalde manier de uitkomsten uit TRUST wel leek te bevestigen.

Bovendien: als een onderzoek(er) zegt: liever niet meteen levothyroxine bij subklinische hypothyreoïdie, wordt hier het onderzoek naar de prullenbak verwezen.
Als een onderzoek(er) het genuanceerder zegt en aangeeft dat er nog meer onderzoek gedaan zou moeten wordne (maar dat je toch alvast beter voorzichtig kunt zijn), is dát hier weer een reden om het onderzoek naar de prullenbak te verwijzen

Prima dat je een paar onderzoeken plaatst die aantonen dat subklinisch hypothyreoïdie een gezondheidsrisico meebrengt.
Maar ten eerste: hoe heb je hier naar gezocht en welke onderzoeken heb je weggelaten?
Ten tweede, ook in de Nederlandse richtlijnen staat dat je subklinische hypothyreoïdie niet a priori en meteen met levothyroxine moet behandelen. Ook maakt de NL-richtlijn extra onderscheid aan de hand van TSH waardes.

En dan kom ik langzamerhand tot de conclusie dat een aantal mensen hier enkel díe onderzoeken accepteren, die hun eigen idee en beeld bevestigen.
Want dit is niet het eerste onderwerp waar dit bij gebeurt. Ook bij het setpoint is er kennelijk maar één juiste model, andere onderzoeken die aangeven dat je bijvoorbeeld ook T3 als variabele mee kunt nemen in de bepaling, worden naar die zelfde prullenbak verwezen. En ook daar is mij inhoudelijk niet duidelijk kunnen maken waarom.

Ten eerste:
ik heb niet naar die laatste artikel (van 14 december) gezocht, ik kwam het tegen toen ik op de site van medical express zocht naar nieuwe info over de schildklier. En daar kwam dit als eerste naar boven.

Ik heb geen onderzoeken weggelaten. Waarom spreek jij mij nu daar op aan?
Via de info op de site van de American College of Cardiology over richtlijnen kwamen andere (gerelateerde !) artikelen aan de zijkant te voorschijn, daarom heb ik die linken ook geplaatst.

Laura, Kiek en meerderen hier op het forum weten dat ik (heel) veel artikelen heb geplaatst, hier als zowel op het oude en ook op het nog oudere voormalig forum van Hypo maar niet Happy. (HmnH)
En dat zijn artikelen breed gevarieerd! Dus niet alleen over subklinisch hypothyreoïdie.


Ten tweede:
- In de afgelopen jaren is er best wel wat veranderd gezien al het nieuws over de schildklier, medicatie etc.
- van Mark
: hoe heb je hier naar gezocht en welke onderzoeken heb je weggelaten?
Welke richtlijnen - wereldwijd - laat jij dan weg?
Jij hanteert alleen de Nederlandse richtlijn en die is 6 jaar oud - uit 2012 - er is nog steeds geen revisie!

Bovendien;
- Waarom zijn er ieder jaar zoveel congressen, bijeenkomsten - wereldwijd - met endocrinologen,
patiëntenorganisaties etc.?
Omdat zij nog steeds nieuwe feiten ontdekken en dit met elkaar delen.

Het artikel wat ik heb geplaatst staat in een medisch journaal over cardiologie, dus over het hart en n i e t in een endocrinologisch journaal.

Het lijkt zo eenvoudig, Mark.
En als je dan leest over een model waarin de T3 - dus beter, denk je dan - en de amateur (zoals jij en ik) veronderstelt dan, dat het wel 'meer waard' zal wezen. Want het draait immers om die T3!
En toch is het juist daarom dat die T3 in de dagelijkse gang van zaken voor de huishouding geen variabele is, maar een referentie.
Want die referentie zít in het set point patroon/huishouding.

Vorige week las ik over de wereldwijde waterhuishoudingsproblemen, overstromingen.
Een hoogleraar uit Delft beschrijft exact een vergelijkbare problematiek. Hoe is het mogelijk dat de uitkomst van zulke ingewikkelde driedimensionale stromingsprocessen op zo'n eenvoudige manier kunnen worden beschreven? Zijn antwoord vond hij in de wijze waarop de natuur een ingewikkeld probleem oplost: altijd de meest efficiënte weg.
Dat is een natuurwet.

Het is in onze schildklierhuishouding niet anders.
En daarom kan het op zo'n verbazend eenvoudige wijze beschreven worden.
Maar niet minder moeilijk.

Die onderzoeken leveren toch geen nieuwe informatie?
En dat gaat al haast vijftig jaar op dezelfde manier.
Het gaat er niet om of jij en ik het interessant vinden, maar of het iets toevoegt aan jouw en mijn kennis en mogelijkheden.

Dit soort onderzoeken heeft dus niets inhoudelijks bijgedragen aan de ontwikkeling en gebruik van levothyroxine, zoals je eerder opmerkte.
Het is het soort onderzoeken dat op het gebied van behandeling van mensen met een hypothyreoïdie geen enkele vooruitgang heeft gegeven.
Het fundament van onze behandeling is dus al 50 jaar oud.

In de jaren zeventig en tachtig zijn er modieuze ideeën overgenomen uit andere disciplines, waardoor vanaf toen nauwelijks fundamenteel onderzoek in ons gebied. De productie van artikelen werd door omstandigheden en bemoeienis van andere partijen belangrijker geacht dan de inhoud.
Je kunt dit lezen in tal van boeken over de ontwikkeling van wetenschappelijk onderzoek.
