A large review of studies has found that vitamin D supplements have little or no benefit beyond the low levels required for bone health.

The meta-analysis, published in The Lancet Diabetes & Endocrinology, combined data from 290 observational studies and 172 random trials.
All the studies used blood levels of vitamin D to measure outcomes. Dosages varied, but most trials used 800 units or more.

The observational studies generally found an association of lower vitamin D levels with increases in cardiovascular disease, lipid concentrations, glucose levels, weight gain, infectious disease and mood disorders.
But random trials showed little or no effect of vitamin D supplements on any of these problems.
The authors conclude that low vitamin D levels are almost surely an effect of these diseases, and not a cause.

Current guidelines recommend supplements for anyone with a blood level under 30 nanograms per milliliter, but the lead author, Dr. Philippe Autier, said that only at levels of 10 or less would there be a risk to skeletal health.
Less than 10 percent of Americans, he estimates, fall into this category.
Dr. Autier is a researcher at the International Prevention Research Institute in Lyon, France.

“Unfortunately, there is probably no benefit to expect from vitamin D supplementation in normally healthy people,” he said.

Summary
Low serum concentrations of 25-hydroxyvitamin D (25[OH]D) have been associated with many non-skeletal disorders. However, whether low 25(OH)D is the cause or result of ill health is not known. We did a systematic search of prospective and intervention studies that assessed the effect of 25(OH)D concentrations on non-skeletal health outcomes in individuals aged 18 years or older. We identified 290 prospective cohort studies (279 on disease occurrence or mortality, and 11 on cancer characteristics or survival), and 172 randomised trials of major health outcomes and of physiological parameters related to disease risk or inflammatory status. Investigators of most prospective studies reported moderate to strong inverse associations between 25(OH)D concentrations and cardiovascular diseases, serum lipid concentrations, inflammation, glucose metabolism disorders, weight gain, infectious diseases, multiple sclerosis, mood disorders, declining cognitive function, impaired physical functioning, and all-cause mortality. High 25(OH)D concentrations were not associated with a lower risk of cancer, except colorectal cancer. Results from intervention studies did not show an effect of vitamin D supplementation on disease occurrence, including colorectal cancer. In 34 intervention studies including 2805 individuals with mean 25(OH)D concentration lower than 50 nmol/L at baseline supplementation with 50 μg per day or more did not show better results. Supplementation in elderly people (mainly women) with 20 μg vitamin D per day seemed to slightly reduce all-cause mortality. The discrepancy between observational and intervention studies suggests that low 25(OH)D is a marker of ill health. Inflammatory processes involved in disease occurrence and clinical course would reduce 25(OH)D, which would explain why low vitamin D status is reported in a wide range of disorders. In elderly people, restoration of vitamin D deficits due to ageing and lifestyle changes induced by ill health could explain why low-dose supplementation leads to slight gains in survival.

Ongoing randomised clinical trials assessing the ability of vitamin D supplementation to reduce the risk of several non-skeletal disorders involve a population larger than that of Cambridge, UK, and will cost millions of research dollars. VITAL, for example, will enroll 20 000 participants and has US$22 million in funding. This vast investment of effort by patients, researchers, and funders is laudable, as it is almost certain that it will be sufficient to answer a question that has long kept the medical community in the dark.

Vitamin D first became a medical success story when its importance in bone health and calcium homoeostasis was proven decades ago. Since then, epidemiological evidence has been accumulating to support a role for vitamin D in the protection of individuals from various non-skeletal disorders including cancer, cardiovascular diseases, autoimmune and inflammatory diseases, dementia, and diabetes; it might also reduce all-cause mortality. Many of these studies show a strong association between low vitamin D concentrations and disease. However, the results of myriad recent small randomised controlled trials are almost unanimous in concluding that vitamin D supplementation provides protection from few, if any, of these outcomes.

Vitamin D is a steroid hormone with pleiotropic and tissue-specific effects owing to the wide expression of the nuclear vitamin D receptor in many different tissues, and the many genes that are targeted by its actions. In the skeletal system, vitamin D promotes healthy development and remodelling of bone. In other tissues, vitamin D is postulated to mediate potentially beneficial effects via a wide variety of mechanisms: some evidence suggests that it exerts anticancer activity by limiting hyperproliferation of certain cell types, that it promotes metabolic health by regulating lipid metabolism in adipocytes, and that it limits autoimmunity by suppressing inappropriate immune responses.

In a systematic review in The Lancet Diabetes & Endocrinology, Philippe Autier and colleagues discuss a large number of observational studies suggesting that high serum concentrations of vitamin D might be protective. For example, those with high vitamin D had decreased risk of cardiovascular events (by up to 58%), diabetes (by up to 38%), colorectal cancer (by up to 33%), and all-cause mortality (by up to 29%). However, they also compare these findings with the results of randomised clinical trials, which reveal a very different picture: no reduction in risk was found, even in trials involving adequate supplementation of participants with low vitamin D levels at baseline (less than 50 nmol/L). Autier and colleagues also did a new meta-analysis of 16 trials that assessed the effects of vitamin D supplementation on blood HbA1c, a biomarker mainly used for monitoring disorders of glucose metabolism. Although type 2 diabetes is associated with low vitamin D, the results show that vitamin D supplementation does not reduce HbA1c. Thus, it looks increasingly likely that low vitamin D is not a cause but a consequence of ill health.

Despite the growing body of evidence indicating that vitamin D is unlikely to prevent non-skeletal disorders, there is strong support for its use from many prominent members of the research community, which is fuelled by the relatively low toxicity of vitamin D, the glimmer of positivity from some trials, and the large body of evidence from prospective observational studies. For those who ‘believe’, the lack of benefit found in most trials completed thus far can be attributed to issues including inadequate supplementation, testing of a population not sufficiently vitamin D deficient at baseline, incorrect formulation, underpowering, or insufficient follow-up. Vitamin D might not be safe in all settings, however. Supplementing at high doses could cause harm in people with already high concentrations of serum vitamin D, particularly in those with liver, kidney, or vascular problems. This is a concern, given the large number of people taking vitamin D supplements (up to 50% of adults in the USA).

Large clinical trials to assess the effects of vitamin D on non-skeletal health outcomes are therefore justified. It would be a real boon to patients if the results are positive, but unless effect sizes for clinically important outcomes are large, the results will only confirm the neutral effect reported by most clinical trials thus far. Although this investment might therefore have little effect on current guidelines, the results will at least allow the research community to move on.

Ondanks honderden onderzoeken is nog steeds geen overtuigend causaal verband aangetoond tussen het geven van extra vitamine D en gunstige gezondheidseffecten. Dat blijkt uit twee onderzoeken gepubliceerd in BMJ.

De afgelopen jaren zouden er steeds meer aanwijzingen zijn dat vitamine D een gunstig effect heeft op aandoeningen als auto-immuunziektes, kanker en cardiovasculaire problematiek. Er blijft echter discussie bestaan over deze effecten.

In het eerste artikel zijn 107 systematische reviews en 151 meta-analyses tegen het licht gehouden. In totaal werden hierin 132 relaties tussen vitamine D en klinische uitkomsten als reumatische activiteit, darmkanker en hypertensie, onderzocht. De onderzoekers stellen dat maar tien van deze associaties gedegen zijn onderzocht en dat voor geen enkele klinische uitkomst overtuigend is aangetoond dat vitamine D een positief effect heeft.

Uit de tweede studie, waaraan het Erasmus MC deelnam, bleek vitamine-D-suppletie, de mortaliteit niet gunstig te beïnvloeden. Voor suppletie met alleen het subtype vitamine D3, werd bij ouderen wel een significant verschil gevonden.

Paul Welsh van de British Heart Foundation en endocrinoloog Naveed Sattar waarschuwen in hun commentaar dat dit geen vrijbrief is om iedereen vitamine-D3 te geven, omdat het ook schadelijke effecten kan hebben zoals hypercalciëmie. Volgens hen moeten artsen kritisch kijken naar de verschillende onderzoeken en niet klakkeloos aan iedereen vitamine-D-suppletie voorschrijven.

En vitamine D in combinatie met Calcium? Vele schildkliermensen hier slikken dat na een operatie, wanneer hun bijschildkliertjes niet meer werken. Ik slik het ook vanwege laag calcium en osteopenie.
Heb je hier ook een artikel over gevonden?

De Gezondheidsraad heeft nieuwe voedingsnormen opgesteld voor vitamine D. Ook adviseert de raad welke groepen mensen gebaat zijn bij inname van extra vitamine D via supplementen. Het advies van de raad is vandaag aangeboden aan de minister van Volksgezondheid, Welzijn en Sport.

Vitamine D is van belang voor een goede botopbouw. Voedingsnormen geven aan hoeveel van een bepaalde stof mensen nodig hebben. De Gezondheidsraad evalueert deze normen regelmatig en past ze zo nodig aan. Daarbij gaat de raad uit van recente wetenschappelijke inzichten. In dit geval heeft de raad gekeken naar aandoeningen waarvan overtuigend is aangetoond of aannemelijk is dat vitamine D daarop een heilzame invloed heeft. Het gaat dan vooral om botgezondheid: de beschermende werking van vitamine D tegen rachitis (bij jonge kinderen), botbreuken en valgevaar (bij ouderen). Het vorige advies van de raad over vitamine D dateert van 2008.

Vitamin D functions in the body through both an endocrine mechanism (regulation of calcium absorption) and an autocrine mechanism (facilitation of gene expression). The former acts through circulating calcitriol, whereas the latter, which accounts for more than 80% of the metabolic utilization of the vitamin each day, produces, uses, and degrades calcitriol exclusively intracellularly.

In patients with end-stage kidney disease, the endocrine mechanism is effectively disabled; however, the autocrine mechanism is able to function normally so long as the patient has adequate serum levels of 25(OH)D, on which its function is absolutely dependent. For this reason, calcitriol and its analogs do not constitute adequate replacement in managing vitamin D needs of such patients.

Optimal serum 25(OH)D levels are greater than 32 ng/mL (80 nmol/L).

The consequences of low 25(OH)D status include increased risk of various chronic diseases, ranging from hypertension to diabetes to cancer. The safest and most economical way to ensure adequate vitamin D status is to use oral dosing of native vitamin D. (Both daily and intermittent regimens work well.)

Serum 25(OH)D can be expected to rise by about 1 ng/mL (2.5 nmol/L) for every 100 IU of additional vitamin D each day. Recent data indicate that cholecalciferol (vitamin D3) is substantially more potent than ergocalciferol (vitamin D2) and that the safe upper intake level for vitamin D3 is 10,000 IU/d.

Vitamine D is een groep van in vet oplosbare prohormonen, waarvan de twee belangrijkste vormen vitamine D2 (ergocalciferol, de plantaardige vorm) en vitamine D3 (cholecalciferol, de dierlijke vorm) zijn. De term vitamine D verwijst ook naar metabolieten en andere analogen van deze substanties. Vitamine D3 kan in het menselijk lichaam worden geproduceerd onder invloed van ultraviolet licht zoals dat van de zon, wat in de klassieke interpretatie van het begrip "vitamine" geïnterpreteerd zou kunnen worden als was vitamine D eigenlijk geen echte vitamine. In de meeste gevallen is de endogene synthese echter ontoereikend en blijft de mens afhankelijk van de aanvoer uit externe bronnen, vandaar dat het in het algemeen toch als vitamine beschouwd wordt. Tegenwoordig wordt vitamine D3 vaak beschouwd als prohormoon, omdat het pas na omzetting (hydroxylatie) in lever en nieren in de actieve vorm (calcitriol) wordt omgezet. Hoewel calcitriol structureel verwant is aan de steroïdhormonen, wordt het geclassificeerd als een secosteroïde omdat een van de koolstofringen open is.

Samenvatting
Een tekort aan vitamine D komt veel voor en kan goed verklaard
worden door een gebrek aan zonlichtexpositie en onvoldoende
vitamine D in voeding en multivitamine preparaten. In risicogroepen is
een bepaling van 25D nuttig om (de mate van) het vitamine D tekort
te objectiveren. Het tekortschieten van de functies van vitamine D is
mogelijk geassocieerd met veel ziekten. Voldoende zonlicht expositie,
zo nodig in samenspraak met de patiënt veilige colecalciferol
suppletie, gericht op een 25D spiegel > 75 nmol/l heeft als doel om bij
te dragen aan de preventie van spierklachten, vallen en chronische
aandoeningen. Tabletten, softgels en drankampullen zijn beschikbaar
om op maat een tekort aan te vullen.

Mijn huisarts zei gisteren: "Wij artsen zijn de bijschildklieren 'n beetje vergeten. De laatste vijf jaar komen ze weer in de belangstelling. Als je over twee jaar aanklopt met 'n primaire hyperparathyreoïdie word je waarschijnlijk onmiddellijk geopereerd..."

Altijd nuchter bloedprikken: niet eten, geen schildklierhormoon slikken en zo vroeg mogelijk.
Dat geeft de betrouwbaarste waarden.
