- Vitamine D-tekort wordt niet langer alleen maar gerelateerd aan het optreden van osteomalacie, rachitis en osteoporose. Er bestaat een causaal verband met de spierfunctie en met het functioneren van ons afweersysteem.
- Daarnaast zijn lage vitamine D-serumwaarden geassocieerd met een verhoogd risico op auto-immuunziekten en diverse typen maligniteiten, zoals prostaat-, colon- en borstkanker.
- Dit heeft geleid tot discussie over de optimale serumconcentratie en aanpassing van de aanbevolen vitamine D-suppletie.
- Er lijkt in Europa voor 25-hydroxyvitamine D consensus te ontstaan over een minimumwaarde van 50 nmol/l en een streefwaarde van 75 nmol/l in het serum.
- Voor bejaarden en allochtonen geldt dat de meerderheid deficiënt of ernstig deficiënt is, waarbij correctie met een stootkuur vitamine D overwogen moet worden.
- Alertheid op een vitamine D-tekort is met name geboden bij onbegrepen spierklachten of -zwakte en bij patiënten uit de risicogroepen, zoals ouderen, allochtonen, chronisch zieken, ‘binnenzitters’, veelvuldig gebruikers van zonnebrandcrème en patiënten met ernstig overgewicht.

Leerpunten
- Voor goede botkwaliteit en spierfunctie is een minimale serumconcentratie 25-hydroxyvitamine D van 50 nmol/l nodig, maar voor preventiedoeleinden wordt een waarde van 75-80 nmol/l aanbevolen.
- Goede voeding of een multivitaminetablet bevat te weinig vitamine D om aan de noodzakelijke behoefte te voldoen.

Kinderen kampen met tekort aan vitamine D

Eén op de drie kinderen in Nederland heeft een tekort aan vitamine D.
Ongezond eten en voor de tv hangen in plaats van buitenspelen zijn de grootste boosdoeners.

Dit blijkt uit een grootschalig onderzoek van het Erasmus MC in Rotterdam onder 4000 kinderen van 6 jaar, dat vandaag wordt gepubliceerd in het wetenschappelijke tijdschrift The Journal of Nutrition.

Bij 6 procent van alle kinderen gaat het om een ernstig tekort. Onder niet-westerse kinderen loopt dit percentage zelfs op tot 20 procent.

Dat het verschil tussen westerse en niet-westerse kinderen zo groot is, komt wellicht door de huidskleur.
Mensen met een getinte of donkere huid maken vanwege de pigmentatie minder snel vitamine D aan.
Maar ook de leefstijl kan een rol spelen.


Zonlicht
Vitamine D is zowel een voedingsstof als een hormoon en zit in voedingsmiddelen zoals vette vis, halvarine en margarine.
Het lichaam kan de stof ook aanmaken wanneer de huid voldoende zonlicht krijgt.
Vitamine D zorgt voor gezonde sterke botten en tanden en de ontwikkeling van het afweersysteem.
Ook verkleint het mogelijk de kans op het ontwikkelen van astma en allergieën bij kinderen.

In Nederland geldt al jaren het advies om kinderen tot 4 jaar extra vitamine D te geven.
Kinderen met een donkere huid of die niet veel buiten komen, zouden ook na hun 4de jaar druppels of tabletjes moeten innemen.
In de praktijk is gebleken dat minder dan de helft van de kinderen supplementen krijgt op 1-jarige leeftijd.
,,We raden ouders aan om hierop alert te blijven,'' zegt voedingsonderzoeker Trudy Voortman.
,,Onze onderzoeksresultaten bevestigen dat het advies van de Gezondheidsraad nodig is.''

Het is voor de eerste keer dat grootschalig onderzoek is gedaan naar de vitamine D-waarden bij kinderen.
,,We hadden niet verwacht dat het om zo veel kinderen met een tekort zou gaan,'' zegt Voortman.
,,Zorgwekkend, omdat vitamine D-tekort op latere leeftijd gezondheidsproblemen kan veroorzaken.''

Bijna één op de drie kinderen heeft te weinig vitamine D.
Bij kinderen van onder andere Marokkaanse, Surinaamse, Turkse en Kaapverdische afkomst gaat het zelfs om meer dan de helft van de kinderen.
Kinderen met een vitamine D tekort eten minder gezond, kijken meer televisie en spelen minder buiten dan kinderen met gezonde vitamine D waarden.
Dat schrijven onderzoekers van het Erasmus MC in het aprilnummer van het wetenschappelijke tijdschrift The Journal of Nutrition.
Zij baseren hun onderzoek op gegevens uit de grootschalige Generation R Studie in Rotterdam.


Vitamine D is zowel een voedingsstof als een hormoon en zit in voedingsmiddelen zoals vette vis, halvarine en margarine, maar het lichaam kan het ook zelf aanmaken wanneer de huid voldoende zonlicht krijgt.
Een getinte of donkere huid maakt minder snel vitamine D aan.
Vitamine D zorgt voor gezonde sterke botten en tanden en de ontwikkeling van het afweersysteem.
Ook verkleint het mogelijk de kans op het ontwikkelen van astma en allergieën bij kinderen.
In Nederland adviseert de Gezondheidsraad om aan kinderen tot vier jaar dagelijks tien microgram extra vitamine D te geven en dit ook na vier jaar te blijven doen bij kinderen met een donkere huid of onvoldoende blootstelling aan zonlicht.
“Onze resultaten bevestigen dat dit advies nodig is, want 30 procent van de zesjarige Rotterdamse kinderen die wij onderzochten heeft te weinig vitamine D in hun bloed”, zeggen auteurs Edith van den Hooven en Trudy Voortman van de afdeling Epidemiologie van het Erasmus MC.

Van den Hooven: “Daar komt bij dat van de niet-westerse kinderen, waaronder kinderen uit Kaapverdië, Marokko, Nederlandse Antillen, Suriname en Turkije, maar liefst 55 procent onvoldoende vitamine D heeft.
Dat wil zeggen een concentratie onder de 50 nmol/L vitamine D in hun bloed.
Bij ruim zes procent van alle kinderen en bijna 20 procent van de niet-westerse kinderen gaat het zelfs om een ernstig vitamine D tekort in het bloed met concentraties onder de 25 nmol/L.
” Kinderen die in de winter waren onderzocht hadden ook veel vaker een vitamine D tekort dan degenen die in de zomer waren onderzocht.

Het verschil tussen de Nederlandse kinderen en kinderen met een andere etnische afkomst is groot.
Dit kan liggen aan hun getinte huidskleur, maar ook aan een andere leefstijl.
Voortman: “Kinderen met voldoende vitamine D waarden keken minder televisie, hadden een gezonder eetpatroon op jongere leeftijd, speelden meer buiten en fietsten vaker naar school dan kinderen met een vitamine D tekort.
Ondanks het advies van de Gezondheidsraad om aan alle kinderen tot vier jaar vitamine D supplementen te geven, kreeg minder dan de helft van de kinderen uit de Generation R Studie op eenjarige leeftijd extra vitamine D. We adviseren ouders om hierop alert te zijn.
Door het advies op te volgen, verkleinen zij het risico op vitamine D tekort bij hun kinderen en mogelijke gezondheidsklachten op latere leeftijd.”
Vervolgstudies moeten uitwijzen of de vitamine D waarden van de kinderen inderdaad samenhangen met specifieke gezondheidsverschijnselen.

De onderzoekers gebruikten de gegevens van meer dan 4000 kinderen uit de Generation R Studie, waarvan de vitamine D concentraties in het bloed werden gemeten op zesjarige leeftijd tussen maart 2008 en januari 2012.
Generation R is een grootschalige bevolkingsstudie naar de groei, ontwikkeling en gezondheid van 10.000 kinderen in Rotterdam.
De kinderen worden vanaf de vroege zwangerschap tot hun jongvolwassenheid gevolgd.

Vitamin D Deficiency in School-Age Children Is Associated with Sociodemographic and Lifestyle Factors

Abstract
Background:
There is concern about a reemergence of vitamin D deficiency in children in developed countries.

Objectives:
The aims of this study were to describe vitamin D status in the Generation R study, a large multiethnic cohort of 6-y-old children in The Netherlands, and to examine sociodemographic, lifestyle, and dietary determinants of vitamin D deficiency.

Methods:
We measured serum 25-hydroxyvitamin D [25(OH)D] concentrations in 4167 children aged 6 y and defined deficiency following recommended cutoffs.
We examined the associations between subject characteristics and vitamin D deficiency with the use of multivariable logistic regression analyses.

Results:
Serum 25(OH)D concentrations ranged from 4 to 211 nmol/L (median 64 nmol/L), with 6.2% of the children having severely deficient (<25 nmol/L), 23.6% deficient (25 to <50 nmol/L), 36.5% sufficient (50 to <75 nmol/L), and 33.7% optimal (?75 nmol/L) 25(OH)D concentrations.
The prevalence of vitamin D deficiency [25(OH)D <50 nmol/L] was higher in winter (51.3%) than in summer (10.3%); and higher in African, Asian, Turkish, and Moroccan children (54.5%) than in those with a Dutch or other Western ethnic background (17.6%).
In multivariable models, several factors were associated with vitamin D deficiency, including household income (OR 1.74; 95% CI: 1.34, 2.27 for low vs. high income), child age (OR 1.39; 95% CI: 1.20, 1.62 per year), child television watching (OR 1.32; 95% CI: 1.06, 1.64 for ?2 vs. <2 h/d), and playing outside (OR 0.71; 95% CI: 0.57, 0.89 for ?1 vs. <1 h/d).
In a subgroup with dietary data (n = 1915), vitamin D deficiency was associated with a lower diet quality, but not with vitamin D intake or supplement use in early childhood.

Conclusions:
Suboptimal vitamin D status is common among 6-y-old children in The Netherlands, especially among non-Western children and in winter and spring. Important modifiable factors associated with vitamin D deficiency were overall diet quality, sedentary behavior, and playing outside.

Prevalence and correction of severe hypovitaminosis D in patients over 50 years with a low-energy fracture

INTRODUCTION

Vitamin D deficiency is common in elderly patients with a fracture caused by a low-energy trauma. Bours et al. found a vitamin D deficiency (< 50 nmol/l) in 64% of their patients, all with a recent fracture.1 Severe vitamin D deficiency is associated with muscle weakness, bone pain, and an increased risk of falls and fractures.2 In general, the supply of vitamin D mainly relies on exposure to the sun, body mass index (BMI) and skin colour.3

The recent Dutch guidelines on osteoporosis and fracture prevention advise a daily intake of 800 IU cholecalciferol for people over 50 years of age and those suffering from osteoporosis. They additionally recommend a 25-hydroxyvitamin D level target value of at least 50 nmol/l.4 However, several authors consider the optimum level to be > 75 nmol/l, since it is considered to be the minimum level to prevent falls.5 All in all, considering its impact on preventing falls and fractures, vitamin D supplementation is of great importance.

No consensus has been reached on whether a post-treatment control level should be established when a mild or severe vitamin D deficiency has been diagnosed, or on what the optimum daily dose of vitamin D supplementation should be in clinical practice.2 Van den Bergh et al. recently proposed to establish a control 25-hydroxyvitamin D level after three months of supplementation and, if necessary, to adjust the recommended dose of cholecalciferol.6 They based their proposal on the finding that, in a low-energy fracture patient population, the optimal level of > 50 nmol/l was often not reached with a daily dose of 800 IU.1

A meta-analysis showed that with a 25-hydroxyvitamin D basal level of < 50 nmol/l, vitamin D supplementation with 400 IU/day led to an average increase in vitamin D levels of 12 nmol/l.5 So far, there have been few reports on the effect of a relatively low dose (400-800 IU a day) of oral vitamin D supplementation on the increase of 25-hydroxyvitamin D levels in patients > 50 years with a (low-energy) fracture. The objective of this study was to examine the following research questions: A: What is the increase of the 25-hydroxyvitamin D level after supplementation with a daily dose of 800 IU of cholecalciferol in patients with a severe vitamin D deficiency (< 30 nmol/l) and a low-energy fracture? B: What percentage of patients will reach the minimum target value of 50 nmol/l? C: Which factors affect the increase of the 25-hydroxyvitamin D level? Factors that were expected to influence this increase were body weight, BMI, renal function, gender, age, season and 25-hydroxyvitamin D basal level.

Vitamine D-preparaat grootste kostenstijger

Groei uitgaven hulpmiddelen met 2,2% naar € 1,5 miljard

Colecalciferol veroorzaakte vorig jaar de grootste stijging in de uitgaven aan extramurale geneesmiddelen: met 60% naar € 35 miljoen. De totale uitgaven bleven met € 4,3 miljard vrijwel gelijk aan 2013. Dat blijkt uit cijfers van het Zorginstituut Nederland.

Het geneesmiddel colecalciferol wordt voorgeschreven bij osteoporose en aan patiënten met een tekort aan vitamine D. Mede door de eind 2012 gepubliceerde NHG-Standaard Fractuurpreventie en de komst van enkele nieuwe producten, was dit vitamine D- preparaat de grootste stijger in de uitgaven. De spécialités Divisun en D-Cura werden het meest voorgeschreven. Het Zorginstituut Nederland concludeert dit op basis van het Genees- en hulpmiddelen Informatie Project (GIP) dat informatie bevat over geneesmiddelen die zijn verstrekt en vergoed volgens de Zorgverzekeringswet. Zorgverzekeraars leveren de declaratiegegevens van in totaal 16,8 miljoen verzekerden aan bij deze GIP-databank.

Verder blijkt dat de uitgaven vorig jaar flink daalden door de patenten die zijn verlopen. Artsen kiezen vaker voor generieke varianten waardoor aan bijvoorbeeld de spécialités CoAprovel en Micardis – twee hypertensiemiddelen – respectievelijk € 10,5 miljoen (52%) en € 6,2 miljoen (57%) minder werd uitgegeven. De kosten voor het migrainemiddel Maxalt liepen terug met 56% naar € 4,8 miljoen.

De invloed van het preferentiebeleid is volgens het Zorginstituut Nederland duidelijk zichtbaar bij de macrogol-combinatiepreparaten. De uitgaven voor deze preparaten zijn met 16% gedaald naar ruim € 38 miljoen, het aandeel van de generieke varianten daarbinnen verdubbelde echter naar € 25 miljoen.


Allergenen
Allergenen werden minder voorgeschreven – een daling van € 12 miljoen – doordat vanaf 2014 slechts een beperkt aantal van deze extracten is geregistreerd. En zorgverzekeraars vergoeden alleen nog de geregistreerde allergenen.

Aan hulpmiddelen is vorig jaar in totaal € 1,5 miljard uitgegeven: een groei van 2,2% ten opzichte van 2013. Vooral de kosten voor hoortoestellen stijgen snel: met bijna 30% naar € 156 miljoen.

Het aantal verzekerden dat gebruikmaakt van hulpmiddelenzorg groeide met 3% naar 2,2 miljoen.


Incontinentiematerialen
De kosten van incontinentiematerialen daalden vorig jaar met 9% naar circa € 149 miljoen. Het aantal gebruikers liep terug met 4,8%; de kosten per gebruiker met 4,2%.

Volgens Zorginstituut Nederland zijn de patiëntprofielen met bijbehorende dagtarieven van zorgverzekeraars mogelijk de oorzaak van deze daling. In een gesprek met de verzekerde moet de zorgaanbieder dit profiel vaststellen.

Het aantal gebruikers van diabetestestmaterialen en het aantal declaraties per patiënt bleef vorig jaar nagenoeg op hetzelfde niveau. Toch daalden de totale uitgaven aan deze materialen met zo’n 13% naar € 87,6 miljoen. Een mogelijk oorzaak van deze daling is het nieuwe inkoopbeleid voor diabeteshulpmiddelen dat enkele zorgverzekeraars in 2014 hebben geïntroduceerd, zo stelt het Zorginstituut Nederland.

A Randomized, Double-Blind, Parallel Study to Evaluate the Dose-Response of Three Different Vitamin D Treatment Schemes on the 25-Hydroxyvitamin D Serum Concentration in Patients with Vitamin D Deficiency

Abstract
Many people worldwide are vitamin D (VTD) deficient or insufficient, and there is still no consensus on the dose of VTD that should be administered to achieve a 25(OH)D concentration of 20 or 30 ng/mL.
In this study, we aimed to determine an adapted supplementation of VTD able to quickly and safely increase the vitamin D status of healthy adults with low 25(OH)D.

One hundred and fifty (150) subjects were randomized into three groups, each to receive, orally, a loading dose of 50,000, 100,000 or 200,000 IU of VTD3 at Week 0, followed by 25,000, 50,000 or 100,000 IU at Week 4 and Week 8.
Whereas 25(OH)D baseline values were not different between groups (p = 0.42), a significant increase was observed at Week 12 (p < 0.0001) with a mean change from baseline of 7.72 ± 5.08, 13.3 ± 5.88 and 20.12 ± 7.79 ng/mL.
A plateau was reached after eight weeks. No related adverse event was recorded.

This study demonstrated a linear dose-response relationship with an increase in 25(OH)D levels proportional to the dose administered.
In conclusion, a loading dose of 200,000 IU VTD3 followed by a monthly dose of 100,000 IU is the best dosing schedule to quickly and safely correct the VTD status.

4. Discussion
Many people worldwide are VTD deficient or insufficient, and there is still no consensus on the dose of vitamin D that should be administered to achieve a 20 or 30 ng/mL vitamin D concentration.

This study was performed to compare three different VTD3 dosing schedules in their ability to increase the 25(OH)D concentration above 20 or 30 ng/mL in subjects who were initially VTD-deficient (?20 ng/mL).

At week 12 of the study, one month after the two month supplementation period with doses of VTD3 corresponding roughly to 1667 to 6667 IU/day, the mean change from the baseline in the different subgroups ranged from +7.7 to +20.1 ng/mL corresponding to 0.30 to 0.46 ng/mL for 100 IU. This is quite similar to an increase of 0.72 ng/mL for 100 IU reported by Heaney [11], but much less than that found by Autier [12] or McKenna [13] who reported in their meta-analyses of interventional studies with different doses of VTD, an increase of 1.96 ng/mL and 2.12 ng/mL for 100 IU vitamin D respectively. It must be noted that in [12,13], the studies that were analyzed most used daily administration of VTD. In the present study, instead of daily doses, loading doses were given in the presence of the study personnel to optimize observance to supplementation. The measurement of the 25(OH)D concentration was also performed one month after each dose. As the 25(OH)D peak concentration after quite a large dose usually occurs within one week after administration and decreases thereafter [14,15], it is likely that, in the present study, the 25(OH)D concentration had already decreased at the moment that the blood samples were obtained, explaining in part the lower increase in the 25(OH)D concentration than in other studies [12,13].

A serum concentration of 25(OH)D >20 ng/mL was obtained for almost all subjects (98%) as early as week 8 in the 400,000 IU VTD3 supplemented group), while 84% and 52% of patients in the 20,000 IU and 100,000 IU VTD3 supplemented groups respectively attained this target only after 12 weeks of treatment. Likewise, more patients attained the target of 30 ng/mL at week 12 in the 400,000 IU VTD3 group (64%) compared to the lower dosage supplemented groups (24% and 4% for the 200,000 IU and 100,000 IU VTD3 supplemented groups, respectively).

These supplementations were demonstrated to be safe as no variation in calcium levels and no clinically relevant adverse events between the groups after three months of supplementation with VTD3 were observed, even with the higher doses. The highest value of 25(OH)D achieved with this protocol was 64 ng/mL at week 8 in a patient in the 400,000 IU VTD3 supplemented group (Table 4).

The main strengths of this study are its double-blind, placebo-controlled design, allowing to control for possible confounders such as season-, or diet-related changes in the 25(OH)D level, and the supervision by the study staff of the administration of all VTD3 doses allowing for a compliance of 100%. Indeed, as the study took place during winter, and as none of the subjects were exposed to tropical or high-altitude sunshine, this suggests that the observed increases in 25(OH)D concentrations were only due to the treatment alone. The study has also some limitations. As the supplements were given under controlled conditions, it should be verified that they can also apply to different categories of patients. In particular, as the subjects included in the study were generally not overweight (BMI 23 kg/m2), and as obese people need more VTD3 than those of normal-weight in order to attain similar levels of 25(OH)D, another study using the same supplementation schemes should be initiated in patients with a BMI >30 kg/m2.



5. Conclusions
In conclusion, this study demonstrated a dose-response relationship presenting a logarithmic pattern between the three patient groups, with an increase in 25(OH)D levels proportional to the dose administered and a plateau after four weeks. The higher the dosing of VTD3, the greater the change in the 25(OH)D serum concentration. A loading dose of 200,000 IU VTD3 followed by a monthly dose of 100,000 IU is, among the doses tested in this study, the best one to quickly and safely correct the VTD status.

Sihl hoopt op 26 juni aan de Vrije Universiteit Amsterdam te promoveren.

Vitamine D-tekort komt veel voor binnen de Nederlandse oudere bevolking. Uit onderzoek blijk dat 50% van de mensen van 65 jaar en ouder een tekort heeft aan vitamine D. Daarom adviseert de Gezondheidsraad aan alle vrouwen ouder dan 50 en alle mannen ouder dan 70 jaar dagelijks extra vitamine D in de vorm van tabletten te nemen.

Sohl keek ook naar wat de mogelijke gevolgen van een vitamine D-tekort zijn. Het bleek dat mensen (>55 jaar) met een lage vitamine spiegel slechter scoorden op verschillende korte fysieke testen en ook ervoeren zij meer beperkingen in zes verschillende activiteiten van het dagelijkse leven zoals teennagels knippen, zichzelf aan- en uitkleden of het gebruiken maken van eigen of openbaar vervoer dan mensen met voldoende vitamine D. Ook hadden deze mensen een groter risico op een toename van de beperkingen na 3 of 6 jaar. Verder bleek een lage vitamine D spiegel een relatie te hebben met een slechtere botkwaliteit. Deze relatie bestond alleen bij mensen met een lage-tot-normale Body Mass Index.

Tevens heeft zij onderzocht of de optimale vitamine D spiegels verschillen voor verschillende subgroepen. Uit deze analyses bleek dat vrouwen, mensen ouder dan 75 jaar en mensen met een hoge Body Mass Index een lagere optimale spiegel hebben dan mannen, 65-75-jarigen en mensen met lage-tot-normale Body Mass Index. Waarom deze groepen een lagere vitamine D spiegel nodig lijken te hebben, zal uit toekomstig onderzoek moeten blijken.

Importance
Experts debate optimal 25-hydroxyvitamin D (25[OH]D) levels for musculoskeletal health.

Objective
To compare the effects of placebo, low-dose cholecalciferol, and high-dose cholecalciferol on 1-year changes in total fractional calcium absorption, bone mineral density, Timed Up and Go and five sit-to-stand tests, and muscle mass in postmenopausal women with vitamin D insufficiency.

Design, Setting, and Participants
This randomized, double-blind, placebo-controlled clinical trial was conducted at a single center in Madison, Wisconsin, from May 1, 2010, through July 31, 2013, and the final visit was completed on August 8, 2014. A total of 230 postmenopausal women 75 years or younger with baseline 25(OH)D levels of 14 through 27 ng/mL and no osteoporosis were studied.

Interventions
Three arms included daily white and twice monthly yellow placebo (n=76), daily 800 IU vitamin D3 and twice monthly yellow placebo (n=75), and daily white placebo and twice monthly 50,000 IU vitamin D3 (n=79). The high-dose vitamin D regimen achieved and maintained 25(OH)D levels ≥30 ng/mL.

Main Outcomes and Measures
Outcome measures were 1-year change in total fractional calcium absorption using 2 stable isotopes, bone mineral density and muscle mass using dual energy x-ray absorptiometry, Timed Up and Go and five sit-to-stand tests, functional status (Health Assessment Questionnaire), and physical activity (Physical Activity Scale for the Elderly), with Benjamini-Hochberg correction of P values to control for the false discovery rate.

Results
After baseline absorption was controlled for, calcium absorption increased 1% (10 mg/d) in the high-dose arm but decreased 2% in the low-dose arm (P = .005 vs high-dose arm) and 1.3% in the placebo arm (P = .03 vs high-dose arm). We found no between-arm changes in spine, mean total-hip, mean femoral neck, or total-body bone mineral density, trabecular bone score, muscle mass, and Timed Up and Go or five sit-to-stand test scores. Likewise, we found no between-arm differences for numbers of falls, number of fallers, physical activity, or functional status.

Conclusions and Relevance
High-dose cholecalciferol therapy increased calcium absorption, but the effect was small and did not translate into beneficial effects on bone mineral density, muscle function, muscle mass, or falls. We found no data to support experts’ recommendations to maintain serum 25(OH)D levels of 30 ng/mL or higher in postmenopausal women. Instead, we found that low- and high-dose cholecalciferol were equivalent to placebo in their effects on bone and muscle outcomes in this cohort of postmenopausal women with 25(OH)D levels less than 30 ng/mL.

Association between low vitamin D and MS

Having low levels of vitamin D doubles the risk of developing multiple sclerosis, an association that researchers conclude supports a causal relationship

Low levels of vitamin D significantly increase the risk of developing multiple sclerosis (MS), according to a study led by Dr. Brent Richards of the Lady Davis Institute at the Jewish General Hospital, and published in PLOS Medicine.
This finding, the result of a sophisticated Mendelian randomization analysis, confirms a long-standing hypothesis that low vitamin D is strongly associated with an increased susceptibility to MS. This connection is independent of other factors associated with low vitamin D levels, such as obesity.

“Our finding is important from a public health perspective because vitamin D insufficiency is common, especially in northern countries like Canada where exposure to sunlight – a common natural source of vitamin D – is decreased through the long winter and where we see disproportionately high rates of MS,” asserts Dr. Richards, who is also an Associate Professor of Medicine and Human Genetics and William Dawson Scholar at McGill University. “We would recommend that individuals, particularly those with a family history of MS, should ensure that they maintain adequate vitamin D levels. This is a common sense precaution, given that vitamin D supplementation is generally safe and inexpensive.”

Adequate intake of vitamin D is defined by the United States’ Institute of Medicine as 600 international units per day for both males and females under the age of 70. Many people, especially in northern climates, may require supplements in order to maintain this level.
The most common permanent neurological disorder affecting young adults

MS, a progressively degenerative disease, is the most common permanent neurological disorder affecting young adults. It is a debilitating autoimmune condition that most often strikes people in their twenties or early thirties. The prospect that vitamin D may serve as a protective measure to prevent its onset is a very exciting development.

The link between vitamin D insufficiency or deficiency and risk of developing MS has been an important area of investigation in the MS research community,” says Dr. Karen Lee, Vice President of Research at the MS Society of Canada. “This research brings us a step closer to understanding whether low vitamin D is a trigger of MS and not just a result of the disease itself. I’m encouraged by the data and hope that it will prompt further research into whether supplementing with vitamin D could reduce the risk or slow the progression of MS.”

By taking the precaution of maintaining a normal level of vitamin D, a person at risk could decrease their risk of acquiring MS by an important degree. “While low vitamin D is by no means the only risk factor, we have identified one risk that can be removed from the equation, which could have a significant impact towards preventing this terrible disease,” concludes Lauren Mokry, who is the first author on the paper and a graduate student at McGill University.

Abstract
Background
Observational studies have demonstrated an association between decreased vitamin D level and
risk of multiple sclerosis (MS); however, it remains unclear whether this relationship is causal.
We undertook a Mendelian randomization (MR) study to evaluate whether genetically lowered
vitamin D level influences the risk of MS.

Methods and Findings
We identified single nucleotide polymorphisms (SNPs) associated with 25-hydroxyvitamin D (25OHD)
level from SUNLIGHT, the largest (n = 33,996) genome-wide association study to date for vitamin D.
Four SNPs were genome-wide significant for 25OHD level (p-values ranging from 6 × 10−10 to 2 × 10−109), and all four SNPs lay in, or near, genes strongly implicated in separate mechanisms influencing 25OHD.
We then ascertained their effect on 25OHD level in 2,347 participants from a population-based cohort,
the Canadian Multicentre Osteoporosis Study, and tested the extent to which the 25OHD-decreasing alleles
explained variation in 25OHD level.

We found that the count of 25OHD-decreasing alleles across these four SNPs was strongly associated with lower 25OHD level (n = 2,347, F-test statistic = 49.7, p = 2.4 × 10−12).
Next, we conducted an MR study to describe the effect of genetically lowered 25OHD on the odds of MS in the International Multiple Sclerosis Genetics Consortium study, the largest genetic association study to date for MS (including up to 14,498 cases and 24,091 healthy controls).
Alleles were weighted by their relative effect on 25OHD level, and sensitivity analyses were performed to test MR assumptions.

MR analyses found that each genetically determined one-standard-deviation decrease in logtransformed
25OHD level conferred a 2.0-fold increase in the odds of MS (95% CI: 1.7–2.5;
p = 7.7 × 10−12; I2 = 63%, 95% CI: 0%–88%).
This result persisted in sensitivity analyses excluding SNPs possibly influenced by population stratification or pleiotropy (odds ratio [OR] = 1.7, 95% CI: 1.3–2.2; p = 2.3 × 10−5; I2 = 47%, 95% CI: 0%–85%) and including only SNPs involved in 25OHD synthesis or metabolism (ORsynthesis = 2.1, 95% CI: 1.6–2.6,
p = 1 × 10−9; ORmetabolism = 1.9, 95% CI: 1.3–2.7, p = 0.002).
While these sensitivity analyses decreased the possibility that pleiotropy may have biased the results, residual pleiotropy is difficult to exclude entirely.

Conclusions
A genetically lowered 25OHD level is strongly associated with increased susceptibility to MS.
Whether vitamin D sufficiency can delay, or prevent, MS onset merits further investigation in long-term randomized controlled trials.

Association of SUNLIGHT SNPs with 25OHD Level
Table 1 displays the four SNPs that achieved genome-wide significance for 25OHD level in SUNLIGHT
and describes their association with 25OHD [19]. Each of these SNPs explained an important
proportion of the population-level variance in 25OHD level, as reflected by the F-statistic.

Vitamin D May Exert Positive Effects by Increasing Dehydroepiandrosterone (DHEA)

I suggest the connection of vitamin D and multiple sclerosis, and other diseases positively affected by vitamin D, is increased dehydroepiandrosterone (DHEA).

It is my hypothesis that low DHEA exposes negative effects of malfunctioning genes involved in MS. (“Multiple Sclerosis: A Theory,” at: http://anthropogeny.com/M... )

It is known that vitamin D receptors are involved in the synthesis of DHEA; it has been found to be involved in bone metabolism: “The study shows that the VDR gene predicts synthesis and/or metabolism of sexual steroid precursor DHEA in parallel with bone mineral density (BMD).” (Horm Metab Res. 2002 Mar;34(3):127-31) I think this relationship will exist in other tissues. Therefore, I suggest low vitamin D levels may be involved in low levels of DHEA.

Just to note that there is an error in the vitamin D pathway - the product of the "italic"DHCR7"italic" gene, 7-dehydrocholesterol reductase, acts to convert 7-dehydrocholesterol to cholesterol, not the other way around as it depicted in the image.

How much vitamin D should I take?

Mokry et al, in a way, answered a question that short term randomized controlled trials (RCT) cannot answer. The authors answered the question, "What happens if I have low vitamin D levels all your life?" The answer is that you double your risk of multiple sclerosis.

Given that fact, how much vitamin D or sunshine should I get to prevent MS? No one knows. The only way to answer that question would be to give pregnant women, their children and then the subsequent adults different physiological doses of vitamin D all their lives and see who gets MS. For obvious reasons, such a study will never be done. In that way, Mendelian randomization studies are more conclusive proof that RCT.

The Vitamin D Council has a simple answer. You should get enough sunshine and/or take enough supplements to have "natural" vitamin D levels. "Natural" vitamin D levels are the levels lifeguards or free-living hunter gatherers in modern day Tanzania have, which are about 50 ng/ml, right in the middle of the reference range for vitamin D, which is 20 - 100 ng/ml.

To obtain such levels from the sun, you have to sunbathe for 10 - 30 minutes/day, not just expose your face and arms. When you can't sunbathe you need to take around 5,000 IU/day of vitamin D3/day. In fact a recent study found that to get 97.5% of all Americans above 20 ng/ml would require 8,000 Iu/day, so 5,000 IU/day is conservative. The Institute of Medicine found that 10,000 IU/day was the No Observed Adverse Effects Level or NOAEL. This means no study has ever found 10,000 IU/day is dangerous.

MS is ot the only Mendelian randomization study to find a link between disease and vitamin D. Total mortality and cancer are also linked to vitamin D deficiency. There are many reasons to have "natural" vitamin D levels.

Volgens mij ben ik dit eerder tegengekomen, maar ik kan het nergens meer vinden ...
Tenminste, het gaat me om commentaar op dit onderzoek.
Het is uitgevoerd in Saudi-Arabië.
Dan denk ik aan mensen die zich beschermen tegen de zon en vrouwen met lichaamsbedekking.

Verband vitamine D-tekort en trage schildklier

Tot op heden is er weinig onderzoek gedaan naar het verband tussen vitamine D en de werking van de schildklier. Recent onderzoek laat echter zien dat de ernst van hypothyreoïdie wel degelijk verband houdt met het vitamine D-gehalte.

Wat nieuw voor mij is, dat niet alleen vitamine D maar ook Calcium wordt geadviseerd.
