In Frankrijk worden momenteel meer dan 2,5 miljoen patiënten behandeld met levothyroxine, voornamelijk als het op de markt gebrachte product Levothyrox®. In maart 2017 is op verzoek van de Franse autoriteiten een nieuwe formulering van Levothyrox® goedgekeurd met het doel om stabiliteitstekorten van de oude formulering te voorkomen. Voordat deze nieuwe formulering werd geïntroduceerd, werd een gemiddelde bio-equivalentieproef uitgevoerd, gebaseerd op door de Europese Unie aanbevolen richtlijnen.

De impliciete redenering was de aanname dat de twee producten, die bio-equivalent zijn, ook omschakelbaar zouden zijn, waardoor substitutie van het nieuwe voor de oude formulering mogelijk zou zijn, waardoor de behoefte aan individuele kalibratie van het doseringsregime van thyroxine wordt vermeden, met behulp van het schildklierstimulerende hormoonniveau als het eindpunt, zoals vereist voor een nieuwe patiënt bij het starten van de behandeling.
Ondanks het feit dat beide formuleringen bio-equivalent bleken te zijn, werden bijwerkingen bij enkele duizenden patiënten gemeld na inname van de nieuwe formulering.

In dit opiniestuk melden we dat meer dan 50% van de gezonde vrijwilligers die deelnamen aan een succesvol regelgevingsgemiddelde bio-equivalentieonderzoek buiten het a priori bio-equivalentiebereik lag.
Daarom betwijfelen we of een gemiddeld bio-equivalentieonderzoek de mogelijkheid biedt om bij patiënten van de nieuwe en oude levothyroxineformuleringen om te schakelen. We stellen verder een analyse van dit probleem voor met behulp van het conceptuele raamwerk van individuele bio-equivalentie.

Dit omvat het onderzoeken van de biobeschikbaarheid van de twee formuleringen binnen een subject, door niet alleen de populatiemiddelen (zoals vastgesteld door de gemiddelde bio-equivalentie) te vergelijken, maar ook door twee variantietermen te beoordelen, namelijk de binnen-subjectvariantie en de variantieschattende subject-by-formulering interactie.
Een hoger binnen individuele variabiliteit voor de nieuwe formulering zou leiden tot heroverweging van de geschiktheid van de nieuwe formulering. Als alternatief zou een mogelijke subject-voor-formulering interactie een beoordeling mogelijk maken van het al dan niet vermogen van artsen om patiënten effectief te beheren tijdens de overgang van de oude naar de nieuwe formulering.


Belangrijke punten
In Frankrijk is een nieuwe formulering van Levothyrox® geïntroduceerd ter vervanging van een bestaande formulering, die niet langer in de handel wordt gebracht, en verwacht wordt dat deze nieuwe formulering ook in de nabije toekomst in alle landen van de Europese Unie op de markt zal worden gebracht, dat wil zeggen dat het dan zal zijn voorgeschreven aan enkele miljoenen patiënten.

Kort na deze verplichte substitutie werden bijwerkingen gemeld aan het Franse netwerk van centra voor geneesmiddelenbewaking, waarbij enkele duizenden patiënten betrokken waren.

Aangetoond werd dat meer dan 50% van de 204 gezonde vrijwilligers, die deelnamen aan een succesvol regulerend gemiddeld bio-equivalentieonderzoek van de Europese Unie, feitelijk buiten het a priori bio-equivalentiebereik lagen.

Er wordt gesuggereerd dat het juiste conceptuele kader om de schakelbaarheid tussen de oude en de nieuwe Levothyrox®-formuleringen te documenteren, individuele bio-equivalentie is.

In France, more than 2.5 million patients are currently treated with levothyroxine, mainly as the marketed product Levothyrox®.
In March 2017, at the request of French authorities, a new formulation of Levothyrox® was licensed, with the objective of avoiding stability deficiencies of the old formulation.

Before launching this new formulation, an average bioequivalence trial, based on European Union recommended guidelines, was performed. The implicit rationale was the assumption that the two products, being bioequivalent, would also be switchable, allowing substitution of the new for the old formulation, thus avoiding the need for individual calibration of the dosage regimen of thyroxine, using the thyroid-stimulating hormone level as the endpoint, as required for a new patient on initiating treatment.
Despite the fact that both formulations were shown to be bioequivalent, adverse drug reactions were reported in several thousands of patients after taking the new formulation.

In this opinion paper, we report that more than 50% of healthy volunteers enrolled in a successful regulatory average bioequivalence trial were actually outside the a priori bioequivalence range.
Therefore, we question the ability of an average bioequivalence trial to guarantee the switchability within patients of the new and old levothyroxine formulations.

We further propose an analysis of this problem using the conceptual framework of individual bioequivalence. This involves investigating the bioavailability of the two formulations within a subject, by comparing not only the population means (as established by average bioequivalence) but also by assessing two variance terms, namely the within-subject variance and the variance estimating subject-by-formulation interaction.

A higher within individual variability for the new formulation would lead to reconsideration of the appropriateness of the new formulation. Alternatively, a possible subject-by-formulation interaction would allow a judgement on the ability, or not, of doctors to manage patients effectively during transition from the old to the new formulation.
