De mogelijke causaliteit van darmmicrobioom in het ontstaan van hypothyreoïdie: IMITHOT studie

Rationale
Bij de ziekte van Hashimoto leidt de aanhoudende lymfocytaire (T-cel) infiltratie tot een irreversibele fibrose van het schildklierweefsel. Training van deze T-cellen vindt onder meer plaats in de (dunne) darm. Eerdere onderzoeken hebben aangetoond dat patiënten met auto-immuun hypothyreoïdie een veranderende samenstelling van het darmmicrobioom hebben. 1-3 .
Van de patiënten met een auto-immuun subklinische hypothyreoïdie met een TSH > 10 mE/L heeft 10-20% binnen 2 jaar progressie van de ziekte van Hashimoto.5 Door juist in deze groep het microboom te veranderen door middel van FMT, hopen wij verdere destructie van de schildklier te voorkomen of uit te stellen door afremmen van de auto-immuniteit van T-cellen.
Studiedesign
De IMITHOT studie is een dubbelblind gerandomiseerd onderzoek, waarin totaal 34 patiënten driemaal een fecestransplantaties ondergaan. Tijdens de follow-up periode van 2 jaar worden de resterende schildklierfunctie, auto-immuunmarkers in plasma en schildklierweefsel, samenstelling van het microbioom, darmpassagetijd en kwaliteit van leven onderzocht.
Inclusiecriteria
• Recent gediagnosticeerde auto-immuun subklinische hypothyreoïdie
• Diagnose < 1 jaar, TSH > 10 mE/L, FT4 12-22 pmol/L, anti-TPO positief
• Geen gebruik van schildklierhormoon;
• Mannen en vrouwen tussen 18 – 70 jaar;
• BMI 18 – 30 kg/m2.

Samenvatting

We willen daarom onderzoeken of microbiële transplantatie van allogene (gezonde) of autologe (eigen) fecale donoren gunstige effecten heeft op de schildklierfunctie bij recent gediagnosticeerde patiënten met auto-immuun hypothyreoïdie.

Opname criteria
- diagnose van auto-immuun hypothyreoïdie - leeftijd 18-70 jaar, BMI 18-30 kg / m2, mannen / vrouwen - antiTPO-positieve en verhoogde TSH (boven 10mE / L) met FT4 binnen normale referentiewaarde.
Uitsluitingscriteria
- aanwezigheid van andere auto-immuunziekten (bijvoorbeeld type 1 diabetes, coeliakie, reumatoïde artritis of inflammatoire darmaandoeningen zoals Crohn / colitis ulcerosa) - antibiotica en PPI-gebruik in de laatste 3 maanden - (verwachte) langdurige gecompromitteerde immuniteit (als gevolg van recente cytotoxische chemotherapie) of HIV-infectie met een CD4-telling <240).

Hypothese
Omdat veranderingen in de samenstelling van gutmicrobiota een rol kunnen spelen bij de ontwikkeling van auto-immuunziekten, waaronder hypothyreoïdie, zullen we onderzoeken of repetitieve fecale transplantatie van allogene (gezonde) of autologe (eigen) fecale donoren gunstig is voor de resterende schildklierfunctie bij recent gediagnosticeerde patiënten met auto-immuun hypothyreoïdie.

Primaire uitkomst
effect op schildklierfunctie zoals bepaald door plasmamarkers (TSH-, T4-, T3- en antiTPO-waarden) evenals resterende schildklierfunctie bepaald door rTSH-test met betrekking tot intestinale microbiota-veranderingen bij aanvang, 6, 12 en 24 maanden.
Secundaire uitkomst
- verandering in auto-immuniteitsmarkers, waaronder FACS op subgroepen van perifeer bloed-lymfocyten en in FNAC-schildklierweefsel (beeldvorming massacytometrie) bij aanvang en na 6 en 24 maanden. - verandering in plasmametabolieten na 0, 6,12 en 24 maanden - verandering in intestinale transittijd (Sitzmark-capsules) zal worden bepaald bij aanvang en na 6 en 24 maanden. - het effect op de vragen over de kwaliteit van leven (ThyPro QoL-schaal) wordt gemeten na 0, 6, 12 en 24 maanden.
